it is called 'the fig sign' and is easy to do. it is a modified fist. you know how to make a 'finger gun' right? index finger extended for the barrel, thumb up for the hammer, pinky ring and middle fingers curled into the palm. well to make a fig first make a finger gun. then just lay your thumb atop your middle finger and curl your index finger into your palm so that your thumb tip pokes out in-between your middle and index fingers. it is a lewd and obscene gesture that has existed in Southern Europe/Turkey and other Mediterranean coastal regions for thousands of years. it is supposed to resemble a female's labia majora and the thumb tip is the clitoris. it basically means, 'having sex'. now why or how the Japanese picked this one up, i'm unsure. maybe it was when Italians would visit Japan during WW2 when they were Axis allies?

another hand sign but a specifically Japanese one is making a fist and extending your pinky finger upwards fully. you show this to the person you're having a conversation with by facing your palm towards the other person. this means, 'i am with a girl' or i am 'occupied with a sexual/romantic partner'...it isn't a male centric hand sign either. once in a blue moon, very very rarely a girl will use this hand sign in anime and manga. :D it is most often depicted when a guy shows up unexpectedly at his male friend's house and the resident opens the front door but shakes his head and shows his buddy 'the sign'. that's a quiet way for the resident of the home to tell his buddy... "shoo! i'm with a girl!"

this hand sign is one of the reasons i laugh so hard at Mike Myers when he's playing Doctor Evil in the Austin Powers movies... when Dr. Evil laughs and does that hand sign, but puts his pinky tip next to the corner of his mouth... heh! that utilization of 'with a girl' basically means, 'she is naked somewhere in the house and you are interrupting us having sex.' :D
